When you switch EHRs without a plan, the work expands to fill whatever time you have — and then spills into your clinical week. Sessions get interrupted by "where did that note go?" moments, and the migration drags on for months. A week-by-week plan is what keeps a solo therapist sane through this.

It spreads a full migration across six weeks, from locking in your foundation to decommissioning the old system. Each week has a clear goal and a short task list you can finish between clients.

How to Use This Plan Template
Copy the weeks below into your calendar or task app. Treat the week numbers as relative — Week 1 is whenever you start, not a specific date. If you have a heavy caseload, stretch each week to two; the order stays the same.
There is one rule that matters more than the dates: do not start a new week until the previous week's goal is genuinely done. Migrations fail when therapists move on before exporting everything or before signing a Business Associate Agreement (BAA).
Week 1: Lock In Your Foundation
Goal: know exactly what you are moving and how you will protect it.
- Read your current EHR contract. Find your export rights, retention terms, and cancellation rules.
- Request a full data export from your current vendor. Do not wait — delivery can take weeks.
- Get a signed BAA from your new EHR. No BAA, no import.
- List your must-have workflows: scheduling, notes, claims, payments, portal, telehealth.
- Make an encrypted, local backup of any records you already have access to.
Week 2: Export and Prepare Your Data
Goal: hold a complete, verified copy of your records outside any vendor.
- Receive and open the export from your current EHR. Confirm it includes clients, notes, appointments, and billing.
- Store the export in encrypted local storage that you control.
- Clean up obvious duplicates or outdated entries you do not need to carry forward.
- Spot-check five random client records to make sure nothing is garbled or missing.
- Keep the old system running. Do not cancel yet.
Week 3: Set Up the New System
Goal: make the new EHR look and feel like yours, before any data moves.
- Create your account and finish security setup: strong password, two-factor authentication, recovery codes.
- Build your note templates — intake, progress, termination — so they are ready to use.
- Set your schedule, appointment types, fees, and reminder rules.
- Connect billing: payers, fee schedule, claim submission, payment processor.
- Turn on the client portal and telehealth, and send yourself a test message.
Week 4: Migrate Records
Goal: move protected health information into the new system without losing anything.
- Import client demographics first. Spot-check ten clients against the export.
- Import clinical records: notes, treatment plans, attachments. Compare a sample line by line.
- Reconcile open balances and upcoming appointments.
- Keep both systems running in parallel.
- Log every access to the export file for your records.
Week 5: Validate in Parallel
Goal: prove the new system works for a real billing cycle.
- Enter all new notes and appointments in the new system; keep the old one read-only.
- Run claims and payments through the new system and confirm they post correctly.
- Catch and fix any gaps you find — missing clients, broken templates, reminder failures.
- Practice your three most common tasks until they feel automatic.
- Do not cancel anything yet.
Week 6: Go Live and Decommission
Goal: switch fully and close the old system cleanly.
- Confirm every active client, open balance, and upcoming appointment is in the new system.
- Cancel the old subscription in writing, citing the retention terms from Week 1.
- Document where the export and backup live, with dates.
- Send clients any new portal or telehealth links they need.
- File your plan and backup locations somewhere you can find again.

A Few Notes for Solo Therapists
