A project plan is different from a checklist or a calendar. A checklist tells you what to do; a calendar tells you when. A project plan frames the whole migration as a sequence of phases with milestones, so you always know which phase you are in and what "done" looks like before you move on.

How to Use This Sample Project Plan
Copy the six phases below into a document, or print this page and check off milestones by hand. Treat each phase as a gate: you do not start the next phase until the current milestone is met. That gating is what turns a stressful migration into a predictable one.
The phases assume a solo practice with one therapist doing the work. If you have a part-time admin or a biller, split the tasks accordingly — the phases and milestones stay the same.
Phase 1: Plan
Goal: decide exactly what you are moving and how you will protect it.
- Define scope: clients, notes, appointments, billing, and documents you will carry over.
- Read your current EHR contract for export rights, retention rules, and cancellation terms.
- Confirm the new EHR will sign a Business Associate Agreement (BAA).
- Identify your must-have workflows and any third-party tools you rely on.
Exit milestone: a written scope and a signed BAA.
Phase 2: Prepare
Goal: secure a complete, verified copy of your records outside any vendor.
- Request a full data export from your current EHR.
- Store the export in encrypted local storage you control.
- Set up the new system's account, security, and templates — but move no data yet.
- Spot-check sample records in the export for completeness.
Exit milestone: a verified, encrypted local backup and a configured new system.
Phase 3: Migrate
Goal: move protected health information into the new system without losing or duplicating anything.
- Import client demographics first; spot-check ten clients against the export.
- Import clinical records, treatment plans, and attachments.
- Reconcile open balances and upcoming appointments.
- Keep both systems running in parallel.
Exit milestone: all records imported and reconciled, old system still active.
Phase 4: Validate
Goal: prove the new system works under real conditions.
- Enter all new notes and appointments in the new system; keep the old one read-only.
- Run claims and payments through the new system for one full cycle.
- Fix any gaps: missing clients, broken templates, failed reminders.
Exit milestone: one clean billing cycle in the new system with no unresolved errors.
Phase 5: Go-Live
Goal: switch fully to the new system.
- Confirm every active client, balance, and appointment is present.
- Send clients any new portal or telehealth links.
- Make the new system your system of record for all new activity.
Exit milestone: the new system is the only place new work happens.
Phase 6: Stabilize
Goal: close out the old system and lock in the new one.
- Cancel the old subscription in writing, citing the retention terms from Phase 1.
- Document where the export and backups live, with dates.
- Finalize your templates and workflows based on what you learned in validation.
- File the project plan and backup locations for future reference.
Exit milestone: old system decommissioned, new system stable for at least two weeks.

Why Phases Beat Rushing
Phases exist so you never have to undo work. If you import records before the new system is configured, you rebuild templates around messy data. If you cancel the old system before validation, you can lose access to records you still need. The gating between phases is the single biggest predictor of a smooth migration.
