Depression narrows life: clients do less, enjoy less, and withdraw from the people and activities that once sustained them. Behavioral activation reverses that by deliberately reintroducing small rewarding and accomplishing actions, even before motivation returns.
This worksheet gives the structure: a week of plan-ahead activities drawn from the client’s own values, plus after-the-fact pleasure and mastery ratings. Over a few weeks the pattern usually becomes clear — certain activities reliably lift mood, and that evidence is what drives the next plan.

What is behavioral activation?
Behavioral activation is an evidence-based treatment for depression. It works on the cycle where low mood leads to withdrawing from rewarding activities, which deepens low mood. The fix is to schedule and complete small meaningful activities even when motivation is low.
Why track pleasure and mastery separately?
Pleasure is enjoyment in the moment; mastery is a sense of accomplishment. Some clients lose one before the other, and tracking both reveals which activities reliably move the needle for that person — useful data for planning the next week.
How big should a planned activity be?
Small. A 10-minute walk or a single text counts. Activities that are too ambitious tend not to happen and reinforce the belief that effort is pointless. Build momentum with easy wins first, then gradually increase difficulty.
Is the 0-10 rating a clinical measure?
No. The pleasure and mastery ratings are simple self-report scales to surface patterns over the week. They complement validated measures like the PHQ-9 rather than replacing them.
Does the worksheet save any client data?
No. All entries stay in the browser until you print or save as PDF. Closing the tab clears the data, which keeps the handout client-confidential.
