Open toolWhat this documentation tracker surfaces
The hardest part of clinical documentation is not writing one note — it is that documentation slowly expands to fill every gap in your schedule. This tracker captures each session and note as a small data point so you can see weekly and monthly trends and notice when note time is creeping past the industry benchmark of about 50% of session time.
Use the tracker to surface template bottlenecks, justify a shorter session length, or make the case for batching and dictation tools. Everything lives in your browser; nothing leaves your device.
Use it for situations like
- You feel like your notes are taking longer and want to confirm whether that is a trend.
- You are choosing between a 45-minute and a 60-minute session and want to model documentation impact.
- You want a defensible record of documentation time for billing or supervision review.
- You are auditing templates to see which note types are the slowest to write.
FAQ
Questions therapists ask before using this calculator
Why track documentation time as a therapist?
Documentation is often the largest hidden cost in a private practice. Logging it makes burnout risks visible, exposes templates that slow you down, and supports honest conversations about session length and fees.
How is the benchmark calculated?
The tracker compares total note time against total session time. The industry rule of thumb is that documentation should not exceed 50% of direct session time. The result shows whether you are on or above that benchmark.
Where is the data stored?
All entries are saved in your browser using localStorage. Nothing is sent to EasyMindCare or any third party, so you can safely use initials or case numbers as labels.
Can I export the entries?
The tracker focuses on live insight in your browser. If you need a CSV for billing or supervision review, the entries live in your localStorage and can be copied into the CEU or referral trackers.
What counts as documentation time?
Include writing the note, checking diagnosis codes, reviewing prior notes, and any small revisions after the session. Skip the session itself but include any note time before the next session.
